Overview
Cagrilintide is a long-acting acylated analogue of human amylin, a 37-amino-acid pancreatic peptide co-secreted with insulin by beta cells of the islets of Langerhans. It has been engineered with fatty acid acylation and amino acid substitutions to extend its plasma half-life to approximately seven days, enabling once-weekly dosing in clinical research settings.
Scientific Overview
Clinical research on Cagrilintide has investigated its effects on appetite, food intake, body weight, and glycaemic parameters, both as a monotherapy and in combination with the GLP-1 receptor agonist semaglutide (under the designation CagriSema). Phase 2 and 3 trials have examined weight reduction in adults with obesity and type 2 diabetes. Preclinical studies have characterised amylin receptor subtypes mediating its central satiety effects and its complementary metabolic actions relative to GLP-1-based therapies.
Mechanism of Action
Cagrilintide activates amylin receptor complexes (AMY1, AMY2, AMY3) -- heterodimers of the calcitonin receptor (CTR) and receptor activity-modifying proteins (RAMP1, 2, 3) -- expressed in the area postrema and nucleus tractus solitarius of the brainstem. Receptor activation suppresses postprandial glucagon release, delays gastric emptying, and promotes satiety through hypothalamic integration. The combination of amylin receptor agonism with GLP-1R agonism provides additive anorectic signalling through partially non-overlapping CNS circuits.
