Overview
PT-141 (Bremelanotide) is a synthetic cyclic heptapeptide and a metabolite of Melanotan II, generated by removal of the N-terminal acetyl group. It is a non-selective melanocortin receptor agonist that acts centrally via the CNS rather than peripherally, and does not produce clinically meaningful changes in skin pigmentation.
Scientific Overview
Research on PT-141 has focused on its central nervous system effects on sexual motivation and arousal pathways. It is the first melanocortin agonist to receive regulatory approval for a sexual dysfunction indication in some jurisdictions. Preclinical studies demonstrated activation of hypothalamic dopaminergic circuits following MC4R agonism. Research has also examined its effects in animal models of social behaviour and motivation, and its distinction from PDE5 inhibitor mechanisms.
Mechanism of Action
PT-141 is a partial agonist at melanocortin receptors MC3R and MC4R, which are expressed in hypothalamic nuclei including the medial preoptic area and paraventricular nucleus -- regions critical to the regulation of sexual behaviour in mammalian models. MC4R activation stimulates dopamine release in the mesolimbic pathway, promoting motivation and arousal independent of peripheral vascular effects. This centrally mediated mechanism distinguishes PT-141 from phosphodiesterase-5 inhibitors, which act peripherally on smooth muscle relaxation.
